To unlock the Balancers boss in The Forsaken Hollows DLC, you need to complete the following prerequisites:

1) Defeat at least two bosses in the main Elden Ring Nightreign campaign — one of them must be Tricephalos.

2) Make both Nightfarers introduced in the DLC, the Scholar and the Undertaker, playable.

Once those conditions are met, go to the chapel that becomes accessible behind the small jar merchant to trigger the Balancers encounter.
